We are happy to verify your plans specific benefits. When you come for your first visit, please have your insurance information available. If your insurance requires that you have a referral form from your primary care physician, you must present it at your appointment. If you do not have the required items, your evaluation may be delayed, and you may be asked to reschedule your appointment. For any billing questions, please contact our office.
