Medical Records Policies

We require written consent signed by the patient or the patient's guardian.

Calls After Hours

Emergency calls after hours are handled by an answering service through our main office. Our doctors are always available to take care of your medical problems.

Other

All minors must be accompanied by parent or guardian.

Any non-medical problems and prescription refills are handled during normal business hours.

Insurance Information

Medicare

We will gladly file an insurance claim for your visit. However, we do not accept Medicare assignment. We ask that you pay for your office visits at the time services are rendered and be reimbursed directly by Medicare.

Worker's Compensation

Complete claim information must be provided and approved prior to the visit.

Managed Care

Our office is contracted with several managed care plans and some are listed below. We welcome each member of these plans and ask that you present an insurance card for verification at the time of your visit. If a referral number is required, the patient is responsible for obtaining the information prior to being seen. If your insurance company requires copayments, they will be collected at the time of service. Be advised that participation in these plans is subject to change. Visit your insurance company’s website or call to verify our participation.

Patient Parking

  • There are several parking lots around the Baylor complex.
    - Garage under Baylor Medical Plaza Towers – enters on Junius
    - Visitors Garage – enters on Junius
    - Valet Parking – enters on Washington in front of towers
    The locations listed above are the most convenient, however, there on other lots and meters around the complex for patient and visitor use.
